Who can request a certificate?

Information from sacramental records is only released to the individual who received the sacrament, the parents of a minor, or to requesting Catholic Church institutions (e.g. parishes or tribunals). 

How can I obtain a copy of a sacramental certificate?

If you are looking for a copy of a Sacramental record, you must fill out the appropriate request form. You will also need to provide a copy of a state-issued photo ID (e.g. Driver's License or passport).

When will the certificate be ready?

Preparation of the certificate will take approximately two weeks.

How much do I have to pay to obtain a copy of a sacramental certificate?

There is no fee to obtain a sacramental certificate; however, due to administrative expenses, a $5.00 donation is suggested and appreciated.
